Kathleen Broglio, NP is currently a Nurse Fellow in the Department of Pain Medicine and Palliative Care at Beth Israel Medical Center in New York City where she is pursuing advanced training in Palliative Care. Prior to this time she worked as a pain management nurse practitioner at a 200 bed culturally diverse hospital in Honolulu. In this capacity, Kathleen conducted patient assessments and provided recommendations for pain management for hospitalized patients. Kathleen also developed pain management education programs for nurses and medical staff. She served on committees both in HI and in the Western Region developing pain management standards, developed and revised pain management related preprinted order sets, and participated in patient pain management quality improvement initiatives. |

Suzanne Bushfield, PhD, MSW, Clinical Associate Professor of Social Work at Arizona State University. Her 30 year social work career includes both the public and private sectors, beginning in public welfare, children and family services, health care, and mental health. She has worked in both inpatient and outpatient mental health services for children, adolescents, and adults. Dr. Bushfield has worked in hospital social work (oncology, nephrology, emergency, home health, and hospice); and in providing treatment to victims and offenders in child sexual abuse. She has had additional training and experience in sex offender treatment, play therapy, grief/loss/bereavement counseling. Before coming to ASU, Dr. Bushfield taught at the University of North Dakota and Lewis-Clark State College in Idaho. |

Daniel Clauw, MD is a Professor of Medicine in the Division of Rheumatology at the University of Michigan . He serves as Director of the Chronic Pain and Fatigue Research Center and Director of the Michigan Institute for Clinical and Health Research, as well as Associate Dean for Clinical and Translational Research within the University of Michigan Medical School. He attended the University of Michigan for both undergraduate and medical school studies and then completed his Internal Medicine residency and Rheumatology Fellowship at Georgetown University. He joined the faculty at Georgetown University in 1990, and while there, founded the Chronic Pain and Fatigue Research Center, and served as Chief of the Division of Rheumatology, Immunology and Allergy, and Vice Chair of Medicine. Dr. Clauw has been committed to the clinical care and research into overlapping conditions such as fibromyalgia, chronic fatigue syndrome, Gulf War Illnesses, and multiple chemical sensitivity. He leads a multidisciplinary team of collaborators at the University of Michigan and other academic medical centers, who are studying the mechanisms and most effective treatments of these illnesses. |

Eliot Cole, MD, MPA, received his undergraduate degree in bacteriology from the University of California, Berkeley; his medical doctorate from the Wake Forest University School of Medicine; and his Master’s degree in Public Administration from the University of Nevada, Las Vegas. He performed residencies in both psychiatry and neurology at the North Carolina Baptist Hospital in Winston-Salem and completed an anesthesia-related fellowship in pain management at the University of California, Los Angeles, Center for Health Sciences. The American Psychiatric Association awarded Dr. Cole fellowship status in 1995 and Distinguished Fellow status in 2002. Presently, Dr. Cole is executive director of the American Society of Pain Educators and a consultant in Pain Management for Kaiser Permanente Medical Group in Honolulu, HI. She received her PhD in clinical psychology at State University of New York at Stony Brook. She specialized in behavioral medicine during internship at the New Orleans Veterans Affairs Medical Center, and during fellowship at Pennington Biomedical Research Center, Louisiana State University. She is active within professional organizations, promoting psychologists involvement in medical settings. Dr. Datz is a member of the American Academy of Pain Medicine, and serves on their Membership Committee. Dr. Duckworth’s clinical and research interests and activities are in the areas of posttraumatic stress and coping consequent to psychological and physical injury. She directs the Motor Vehicle Collision and Chronic Pain Research Program at the University of Nevada, Reno. She conducts laboratory-based and clinical research that examines the pre-, peri-, and post-collision variables that predict physical and psychological functioning consequent to an injury-causing collision and she provides direct clinical service to adults who are experiencing chronic pain, functional impairment, and traumatic stress reactions as a function of motor vehicle collision involvement. In collaboration with Drs. Tony Iezzi and William O’Donohue, Dr. Duckworth is currently preparing an edited volume entitled Motor Vehicle Collisions: Medical Psychosocial, and Legal Consequences. |

Dr. Glick currently practices in Richmond, Virginia, where he concentrates on pain associated with the spine and extremities primarily within model of interventional pain management. While earning his degree, he had been involved with research relying upon evoked potentials. This led to his clinical practice utilizing evoked potentials to guide interventional pain treatments. He has since lectured on clinical applications and improving evoked potential techniques, as well as differential diagnosis of repetitive stress injuries, difficult neck and back pain patients including post-operative pain syndromes. Howard Heit, MD is board certified in internal medicine (diplomat) and gastroenterology (diplomat). He is also certified in addiction medicine by the American Society of Addiction Medicine (ASAM) and is certified as a medical review officer (MRO). He is a fellow of the American College of Physicians and the American Society of Addiction Medicine. Dr. Heit is also a member of the American Academy of Pain Medicine (AAPM) and the American Pain Society (APS). Dr. Heit was co-section coordinator, author and an editor on “Pain Management and Addiction Medicine” for ASAM’s textbook Principles of Addiction Medicine, Second and Third Edition. Dr. Heit served as chair of ASAM’s scientific program “Common Threads: Pain & Addiction,” from 2000 - 2005. He is also a member of the AAPM annual scientific meeting planning committee. From 1990-1996, Dr. Heit served as medical addictionologist for Alcohol and Drug Services for Fairfax County. Dr. Heit is an assistant clinical professor of medicine at Georgetown University School of Medicine. Dr. Heit has lectured and published extensively on the interface of pain and addiction medicine. Dr. Heit (current chair) was also instrumental in forming the Liaison Committee on Pain and Addiction (LCPA) of the American Society of Addiction Medicine, the American Academy of Pain Medicine, and the American Pain Society. The mission of this committee is to encourage collaboration between pain specialists and addiction specialists on issues of common interest, including areas of research, education, clinical care and policy development. |

Barbara L. Kornblau, JD, OT is Professor, Occupational Therapy, Public Health, & Law Nova Southeastern University. Dr. Kornblau is the immediate Past President of the American Occupational Therapy Association (AOTA), and a Professor of Occupational Therapy, Public Health and Law at Nova Southeastern University where she teaches course work in pain management. In her private practice, she provides consulting services to business and industry, attorneys and others for purposes of complying with the Americans With Disabilities Act (ADA). A practicing attorney, she specializes in disability discrimination litigation and advocacy for individual experience chronic pain. She is also a certified case manager, a certified disability management specialist and holds board certification in pain management and disability analysis. Dr. Kornblau has published over 100 journal articles, books, book chapters and other works on the Americans With Disabilities Act, ethical issues in health care, accommodating students with disabilities, medico-legal issues, consultation, pain management, functional capacity evaluations, occupational therapy intervention with various populations, and work rehabilitation. Bill H. McCarberg MD is Founder of the Chronic Pain Management Program for Kaiser Permanente in San Diego, California. He was on the board of directors of the American Pain Society and current board of directors of the National Pain Foundation. He is president of the Western Pain Society and Assistant Clinical Professor (voluntary) at the University of California at San Diego School Medicine. Dr McCarberg is a member of the American Academy of Family Physicians, the American Academy of Pain Medicine, the American Pain Society, and the International Association for the Study of Pain. He is the recipient of several awards, including the Shilling Compassionate Care Award, and in 1998 was named the Highest Rated Physician by Member Appraisal of Physician Services at Kaiser Permanente. He also received the Elizabeth Narcessian award for leader in the field of pain education from the American Pain Society. He has given more than 200 presentations on pain management issues and is the author or co-author of several publications. He is board certified by the American College of Pain Medicine, the American Board of Family Practice and additionally certified in Geriatrics. Dr McCarberg received his MD degree from Northwestern University Medical School in Chicago, Illinois. Mary Lynn McPherson, PharmD, BCPS, is Professor at the Department of Pharmacy Practice and Science School of Pharmacy, University of Maryland. Mary Lynn McPherson, PharmD, BCPS, received her Doctor of Pharmacy degree from the University Of Maryland School Of Pharmacy in 1986, where she is currently on faculty as an Associate Professor in the Department of Pharmacy Practice and Science. Dr. McPherson has maintained an active practice in hospice and palliative care for over 18 years. She consults with several hospice programs in the Baltimore area, including Seasons VNA Hospice and Palliative Care of Maryland, Hospice of the Chesapeake and Heartlands Hospice. Dr. McPherson teaches extensively on pain management and palliative care at the University Of Maryland School Of Pharmacy. This includes didactic content, small discussion groups, and case management discussions. She serves as course masters of electives including "Care of the Terminally Ill," "Grief and Bereavement Theory," and a course coordinated with the American Cancer Society titled, "I Can Cope: Pharmacist Educators in Pain Management." Dr. McPherson is passionate about teaching practicing pharmacists about pain management and end-of-life, and she is currently developing a certificate program in this area. She is a frequent lecturer on a local, state and national level to pharmacy, nursing and medical groups on pain and symptom management. Dr. McPherson developed the first palliative care pharmacy residency in the United States, and has published many journal articles on pain management and palliative care, as well as other topics, several books and numerous book chapters. |

Russell K. Portenoy, MD is Chairman, Department of Pain Medicine and Palliative Care, and Chief Medical Officer of Continuum Hospice Care/The Jacob Perlow Hospice. He holds the Gerald J. and Dorothy R. Friedman Chair in Pain Medicine and Palliative Care at Beth Israel Medical Center and is Professor of Neurology and Anesthesiology at the Albert Einstein College of Medicine. Dr. Portenoy received his medical degree from the University of Maryland School of Medicine, and completed a residency in Neurology at the Albert Einstein College of Medicine and a Fellowship in pain management at Memorial Sloan-Kettering Cancer Center. Dr. Portenoy is Past President of the American Pain Society and formerly Councillor and Secretary of the International Association for the Study of Pain. He is President-Elect of the American Academy of Hospice and Palliative Medicine, and is currently Chairman of the American Board of Hospice and Palliative Medicine, Chairman of the Medical Forum of the National Hospice Work Group, and Chairman of the Committee on Hospice and Palliative Medicine of the Accreditation Council on Graduate Medical Education. He serves on the Board of Directors of the American Pain Foundation. Dr. Portenoy is the recipient of the Wilbert Fordyce Award for Lifetime Excellence in Clinical Investigation and the Distinguished Service Award from the American Pain Society, the Founder’s Award from the American Academy of Pain Medicine, and the Bonica Award from the Eastern Pain Association. His research has focused on clinical trials of analgesic drugs, opioid pharmacology, symptom measurement, and quality of life assessment. He is Editor in Chief of the international, peer-reviewed Journal of Pain and Symptom Management; Associate Editor for Clinical Sciences of Pain; and Editor for the Palliative Care Section of The Oncologist. He serves on numerous other Editorial Boards and has written, co-authored or edited 17 books and more than 450 papers and book chapters on topics in pain and symptom management, opioid pharmacotherapy, and symptom assessment. |

Andrea Rapkin, MD

Andrea Rapkin, MD received her undergraduate degree from Cornell University in 1975. She went on to study medicine at the University of New York in Buffalo where she received her M.D. in 1979. Dr. Rapkin completed a residency in Obstetrics and Gynecology at UCLA School of Medicine in 1983. She is Board Certified by the American College of Obstetricians and Gynecologists (of which she is also a fellow). After obtaining her M.D., she joined the faculty at UCLA and is currently a Professor of Obstetrics and Gynecology. She also directs the UCLA Pelvic Pain Clinic. Dr. Rapkin has published extensively in the areas of premenstrual syndrome, chronic pelvic pain and women's reproductive health, with numerous research articles and contributions to textbooks in the area of menstrual-cycle-related mood disorders and pelvic pain. Dr. Rapkin was one of the first Obstetrician-Gynecologists to adapt the multidisciplinary pain management approach to the evaluation and treatment of women with pelvic and vulvar pain. The clinic includes professionals from the areas of psychology, anesthesiology, and physical therapy and provides patient care, resident medical student education, and a fruitful environment for clinical research. |

Michael E. Schatman, PhD has practiced as a clinical psychologist in pain management for over 20 years. He is the author of numerous journal articles, book chapters, and presented papers on pain management and medical bioethics. Additionally, Dr. Schatman is the editor of both Ethical Issues in Chronic Pain Management and Chronic Pain Management: Guidelines for Multidisciplinary Program Development. He serves on the Board of Directors of the Western Pain Society, and is an Editorial Board member of Practical Pain Management. He is currently in consulting practice in Bellevue, WA. |

Peter S. Staats, MD, MBA, is Adjunct Associate Professor in the Department of Anesthesiology and Critical Care Medicine and Department of Oncology at The Johns Hopkins University School of Medicine in Baltimore, Maryland. He is also Chief Executive Officer of American Pain Medicine, Inc., and a Pain Management Physician at Metzger Pain Management in Shrewsbury, New Jersey. Dr. Staats earned his MD from The University of Michigan Medical School, did his residency and was Fellow in Pain Medicine at The Johns Hopkins University School of Medicine, and received his MBA from The Johns Hopkins University. He is a Fellow of the World Institute of Pain. Dr. Staats has received numerous honors and awards for his work in pain medicine, and is widely published on pain theories, spinal cord pain, and management of cancer-related pain. He holds many editorial board appointments, including Executive Editor of Pain Practice, and Associate Editor of both Pain Medicine and The Clinical Journal of Pain, among other roles. Dr. Staats has been the Principal Investigator for several sponsored clinical trials on pain, helping to advance the information base on pain and its treatment. He is very active in professional organizations and societies, in the areas of both pain and anesthesiology. |

Dennis C. Turk , PhD, is the John and Emma Bonica Professor of Anesthesiology and Pain Research and director of the Fibromyalgia Research Center at the University of Washington. He serves as a special government employee within the Food and Drug Administration. Prior to his current position, Dr. Turk was the director of the Pain Evaluation and Treatment Institute and a professor of psychiatry and anesthesiology at the University of Pittsburgh, and an associate professor at Yale University. Dr. Turk was formerly the editor in chief of The Annals of Behavioral Medicine and Pain Management Today, and he is currently the editor in chief of The Clinical Journal of Pain. He is the co-coordinator of the Initiative on the Methods, Measurement, and Pain Assessment in Clinical Trials (IMMPACT). Dr. Turk has published over 425 articles and chapters in scholarly texts on areas associated with assessment and treatment of chronic pain and authored or edited 13 volumes on these topics. |

Gary A. Walco, PhD is a Professor of Pediatrics at UMDNJ, New Jersey Medical School and Director of the David Center for Children’s Pain and Palliative Care at Hackensack University Medical Center. He has conducted multiple studies on pain management and palliative care in children and has published over 90 journal articles, book chapters, and abstracts. Dr. Walco was the founder of the Special Interest Group on Pain in Infants, Children, and Adolescents for the American Pain Society and was their 2003 recipient of the Jeffrey Lawson Award for Advocacy in Children’s Pain Relief. He chaired the task force responsible for the American Academy of Pediatrics policy statement on the assessment and management of acute pain in children, and currently is on the core committee for Palliative Care in the Children’s Oncology Group, is the co-chair of the Pain Committee for the Children’s Arthritis and Rheumatology Research Alliance, is an invited member of the Neonatal Pain Research Initiative (NICHD, FDA) and the Pediatric IMMPACT group, and serves as a consultant to pharmaceutical firms on analgesic trials for children. He is on the editorial boards of the Clinical Journal of Pain, the Journal of Pain, the Journal of Developmental and Behavioral Pediatrics, and the Journal of Pediatric Psychology. |

Arthur L. Weaver, MD

Arthur Lawrence Weaver, MD, MS (Medicine), FACP, MACR, is Clinical Professor in the Department of Internal Medicine at the University of Nebraska Medical Center in Omaha. He is board certified in internal medicine and rheumatology. Completing his medical degree at Northwestern University Medical School, Chicago, Illinois, Dr. Weaver interned at University of Michigan Hospitals in Ann Arbor. He completed his residency and fellowship in internal medicine and rheumatology at the Mayo Graduate School of Medicine, Rochester, Minnesota, and received his Masters Degree in Medicine from the University of Minnesota. Dr. Weaver has been extremely active in medical organizations for rheumatology. He is a Fellow of the American College of Physicians and the American Rheumatism Association, and a Master of the American College of Rheumatology. His contributions to the field of rheumatology are vast: He has been the primary investigator for more than 115 clinical research protocols; he has contributed to the literature with a wealth of articles and abstracts; and he has lectured extensively nationally and abroad. |
